About Postpartum Hemorrhage (PPH) and the Need for Research

Postpartum hemorrhage (PPH) is a significant global health challenge and a leading cause of maternal mortality worldwide. Women who experience PPH are at risk of severe blood loss, which can lead to long-term health complications or even death. While effective treatments for PPH exist, access to these treatments is often limited in low- and middle-income countries (LMICs), particularly in regions with inadequate health infrastructure and resources.

The current standard treatment for PPH is oxytocin, a uterotonic medication that helps to contract the uterus and control bleeding. However, oxytocin requires continuous refrigeration to maintain its potency, posing challenges for storage and transportation in settings with unreliable cold chain systems. As a result, many women in LMICs do not have access to high-quality oxytocin, increasing their risk of adverse outcomes during childbirth.

To address this critical gap in maternal health care, WHO is conducting a multi-country randomized clinical trial to evaluate the efficacy and safety of heat-stable carbetocin (HSC) as a first-line treatment for PPH. Unlike oxytocin, HSC does not require refrigeration, making it more suitable for use in resource-limited settings where cold chain infrastructure may be lacking. If successful, this trial could revolutionize the management of PPH and significantly reduce maternal mortality worldwide.
